Linkage-to-Care

Linkage-to-Care assists newly diagnosed patients, patients falling out of care, patients transferring care to Matthew 25, and patients transferring care away from Matthew 25 receive prompt medical intervention by gaining access to specialty services.

Early treatment of HIV has many benefits, such as improved health outcomes, opportunities for risk reduction interventions, and lower medical costs.

The Linkage-to-Care Specialist will:

  • Build effective, working relationships with each patient.
  • Help the patient to develop acceptance of their HIV positive status.
  • Address questions and concerns about managing HIV.
  • Create and execute plan for overcoming barriers to care.
  • Coordinate and link each patient to community resources based on patient’s needs.

*Anyone diagnosed with HIV/AIDS and not currently under medical care is eligible for Linkage-to-Care services.

All Matthew 25 patients can use our Patient Portal to manage care at Matthew 25.

Patient Confidentiality

At Matthew 25, you have the full right to confidentiality in your health care records which includes identifying information such as disclosed LGBTQIA+ status. Our providers have a moral and legal obligation to maintain this confidentiality and not disclose any information without prior consent.

Patient No-Show/Cancellation Policy

Matthew 25 understands that there will be times when you may need to cancel your clinic appointments (including appointments for support services), and that sometimes happens without you giving us proper notice. However, we would appreciate it if you could give at least 24+ hours notice whenever you are aware of a need to reschedule your appointment so that we can offer that time to another patient who may be waiting until a later date to be seen. If the clinic is closed, please call the location for your appointment and leave a confidential voicemail.

After Hours Service

Matthew 25 will meet after after-hours medical needs of patients by offering after-hours services. A clinical staff member is available by phone 24 hours a day, 7 days a week.

If there is no answer, please leave a voice message including your name, number (area code included), and date of birth. Your call will be returned as timely as possible.

Access to Health Information

Matthew 25 offers you a secure way to easily access your medical information online, so you may view your health record at any time or place through an app called Healow.

You will be given a username and password and will receive periodic updates through the personal e-mail address on file.

You only need to provide a personal e-mail address in order to view your personal health record from a smartphone or any computer with an internet connection.

Patient Navigation

Our patient navigation provides linkage to patients entering into care. Our Patient Navigator also provides assistance with verifying insurance, enrollment, or identifying patient assistance programs that may be available.
