Matthew 25 also includes Case Management Services at each of our locations. With this service, patients are assigned to a specific case manager who then works with them to coordinate their treatment and health care services to improve their well-being and quality of life. Medical Case Management can assist with:

· Transportation
· Co-pay Assistance
· Patient Assistance Programs
· Referrals from Providers
· Medication Adherence
· Food Pantry & Nutritional Supplementation

Indiana

Enrolling in MCM services in our Indiana clinic looks a little different than our Kentucky clinic locations. Our IN MCM staff works closely with the AIDS Resource Group, ARG, to provide a wrap-around service for enrollment in Ryan White-funded programs. Though all patients will have access to medical case management staff for assistance, patients who do not enroll in Ryan White services with ARG will not be assigned to a specific MCM staff member.

  • Personalized Case Management – we work with every MCM patient to build a care plan that is unique to each of their personal situations and provide support to help the patient achieve their set goals.
  • Insurance Navigation – working with the Indiana Department of Health, our MCM/Patient Navigator can help link new patients to insurance enrollment services.
  • Medication/Treatment Management – working with the clinical team, MCM staff help patients to keep their treatment plans and medication adherence as a top priority.
  • Community Resource Connection – MCM’s help connect patients with the community options they are needing to gain access to.
  • Co-Pay/Patient Assistance Programs – medication access is a top priority for all of our patients. Our MCM team can help with navigating medication costs by enrolling patients in the medication manufacturer assistance programs.
  • Transportation – Matthew 25 provides all of our patients with access to transportation services including staff transport in agency vehicles, gas cards and bus tokens for the METS bus system in Evansville.
  • Food Pantry/Nutritional Services – our IN clinic has a pre-constructed food options, from Zack’s Kitchen, that can help to offset the cost of groceries to our patients. Our MCM staff can also work with the providers to advocate for patients wishing to receive nutritional supplements, such as Ensure.
